Failure Modes and Effects Analysis (FMEA)/Critical Items List (CIL).

The FMEA is a bottom-up analysis of component-level failures and their effects on higher-level systems; it is usually performed by the assigned reliability office to identify critical items in hardware. The FMEA should be used to assist safety personnel to perform hazard analyses and supplement, not replace, hazard analyses.

De Failure Mode and Effect Analysis is een analysemethode waarbij een voorspelling wordt gemaakt van het toekomstige gedrag van een product of proces. Met FMEA hebt u een belangrijk instrument in handen als u kwaliteit in het ontwikkelingstraject wilt integreren. Deze methodiek kan een belangrijk hulpmiddel zijn om een kennissysteem (op basis van ervaring en gekoppeld aan product- en procesmodules) op te zetten. Hierdoor worden kostbare verbeteringen en corrigerende acties tijdens de productie en het gebruik vermeden.

Im Rahmen des vom BMBF geförderten Verbundprojekts INDIA wurden zusammen mit der Robert Bosch GmbH Verhaltensmodelle für Komponenten mechatronischer Fahrzeugsubysteme entwickelt. Unter Verwendung dieser Modellbibliotheken und eines vorhandenen Softwaretools zur automatischen modellbasierten Verhaltensvorhersage soll im Rahmen dieser Arbeit ein Prototyp zur Unterstützung der FMEA realisiert werden. Teilaufgaben sind dabei

- die automatische Ableitung des Systemverhaltens für alle möglichen Einfachfehler von Komponenten,

- die Ermittlung der Unterschiede zum Normalverhalten des Systems,

- die computerunterstützte Generierung einer FMEA-Tabelle auf Grundlage dieser Ergebnisse.

Dieser FMEA-Prototyp soll in MS Visual Basic unter Windows NT implementiert werden.

RQM-FMEA ist ein Baustein zu Erstellung von FMEAs (Fehler-Möglichkeit- und -Einfluss-Analyse), die zur Ermittlung von Fehlermöglichkeiten am Produkt (System-FMEA Produkt, Konstruktions-FMEA) oder im Herstellprozess eines Produkts (System FMEA Prozess, Prozess-FMEA) verwendet wird.

Unter Zuhilfenahme der so (oder manuell) erstellten FMEA können erkannte Merkmale, die der Prüfung bedürfen definiert und in der Prüfplanung übernommen werden.

zu entdecken und Maßnahmenpläne zu ihrer Beseitigung zu erstellen. Deshalb wird sie

In een FMEA worden van ieder onderdeel uit een produkt (of iedere stap uit een produktie-proces) de functies benoemd.
Van iedere functie worden mogelijke faalmanieren geformuleerd.
Vervolgens worden voor iedere faalmanier drie grootheden geschat:
de kans op optreden (O),
de ernst (E)
en de kans op het onopgemerkt blijven van de fout (D van detecteerbaarheid).
Deze drie grootheden worden met elkaar vermenigvuldigd en leveren aldus het 'risico-prioriteits-nummer' op.
Het RPN wordt hoger naarmate het optreden van de fout waarschijnlijker is, meer schade veroorzaakt en moeilijker tijdig te signaleren is.
Hoe hoger het RPN, hoe urgenterer actie ondernomen moet worden om het optreden van de potentiële fout te voorkomen.

Over the last few years, there have been several industry initiatives to develop risk-based inspection programs as alternatives to currently required ASME Code, Section XI Inservice Inspection (ISI) programs. These efforts have been initiated by owners groups, the Electric Power Research Institute and ASME Boiler and Pressure Vessel Code Subcommittee XI. A working group has been established under Subcommittee XI to study the application of risk-based technologies to ISI and to develop the necessary Code changes to permit their use in lieu of current requirements. There are also a number of ongoing industry risk-based pilot studies to demonstrate the feasibility and effectiveness of the proposed methods.

The overall goal of these efforts is ultimately to allow the use of risk assessment, plus an understanding of component-specific degradation mechanisms, to establish an effective plant integrity management program which maintains plant safety, while at the same time reducing the industry and regulatory burden associated with current ISI requirements. Preliminary results from the pilot studies have shown that the application of risk-based techniques will allow nuclear plants to reduce the examination scope of current ISI programs by as much as 60% to 80%, without sacrificing safety. A brief discussion of the ongoing programs will follow.
